Documents required submission for Preschool enrollment
The Irvine Chinese Immersion Preschool is licensed and authorized by the California Department of Social Services to offer daycare for preschool age children. Certain forms are required by the CALIFORNIA DEPARTMENT OF SOCIAL SERVICES as part of the admissions process. Below is a list of essential documents required submission for the enrollment process to be finalized.
Note: Forms D1 to D10
- Forms D1, D2, D3, D4, D5, D8, and D10 could be filled in and submitted via the Online Forms. After you submit the online form, a copy will be emailed to you for your records.
- If you prefer, you can download the PDF forms, print them out, fill them in, and then bring the physical copies to the school.
- Microsoft Edge makes it easy to fill in blank PDF fields without printing. Simply open the PDF in Edge, type directly into any fillable field, and save. For detailed guidance, please see the instructions provided below.

How to use Microsoft Edge PDF Reader

Did you know that you can also fill-in the blank fields on a PDF using Microsoft Edge? Instead of printing the form, filling it out by hand, and scanning it back to your PC, fill out the PDF’s blank fields directly in your browser. Here's how:
- Open the PDF in Microsoft Edge.
- Select Edit in the toolbar at the top of the screen.
- Select the blank fields you want to edit and type in your text.
- Save the edited PDF by selecting Save in the top toolbar.
It's that simple! You can also add new text, images, and shapes to the PDF by selecting Add notes in the top toolbar.

There might be times you’ll want to save your PDF after you’ve made edits and printed a paper copy. To save a PDF in Microsoft Edge:
- Open the PDF in Microsoft Edge.
- Select Save in the toolbar at the top of the screen.
- Choose where you want to save the PDF and give it a name.
- Select Save and your PDF will be saved to the location you selected.
Sign the Document
- On the top toolbar, click the Draw icon (which resembles a pen or pencil).
- Select your preferred ink color and pen thickness from the options.
- Use your mouse, trackpad, or touchscreen to draw your signature directly onto the signature line.
- Tip: Zoom in on the document to 150% or 200% for better precision if you are using a mouse. [1, 2, 3, 4]
4. Save the Signed PDF
- Click the Save icon (floppy disk) or Save as in the top-right corner to save your changes to the original file or create a new copy. [1, 2, 3]
