Documents required submission for Preschool enrollment

The Irvine Chinese Immersion Preschool is licensed and authorized by the California Department of Social Services to offer daycare for preschool age children. Certain forms are required by the CALIFORNIA DEPARTMENT OF SOCIAL SERVICES as part of the admissions process. Below is a list of essential documents required submission for the enrollment process to be finalized.

Note: Forms D1 to D10

  • Forms D1, D2, D3, D4, D5, D8, and D10 could be filled in and submitted via the Online Forms. After you submit the online form, a copy will be emailed to you for your records.
  • If you prefer, you can download the PDF forms, print them out, fill them in, and then bring the physical copies to the school.
  • The Physician will provide a copy of forms D6 and D7.
