2026 LEGENDS Production Forms
The Irvine Chinese Immersion Preschool is licensed and authorized by the California Department of Social Services to offer daycare for preschool age children. Certain forms are required by the CALIFORNIA DEPARTMENT OF SOCIAL SERVICES as part of the admissions process. Below is a list of essential documents required submission for the enrollment process to be finalized.

- Forms D1, D2, D3, D4, D5, D8, and D10 could be filled in and submitted via the Online Forms. After you submit the online form, a copy will be emailed to you for your records

2026 Performance Contract
A representative from each performing group must sign the 2026 Performance Contract on behalf of the performing group. Individual performers not participating as part of a performing group must also sign the 2026 Performance Contract.
